In the Jan 6, 1997 issue of PC Week, Spencer F. Katt has this bit:

"After Version 4.5 of the company's [Lotus] ubiquitous Notes went into
manufacturing, there were red faces aplenty when the quality control
folks belatedly discovered that the product documentation did not fit
into the box."
